Hi, I'm Mahd Nishtar.

I'm an interventional and cancer pain physician. I trained at Columbia University Irving Medical Center and NewYork-Presbyterian. I founded an inpatient pain service at my hospital, and I direct pain medicine education for residents and medical students.

Here I write about attention, health and Great Work. You can get my best ideas every Sunday in the 1-3-1 newsletter. Click here to learn more and sign up.

You can find me elsewhere on X and Substack.

How My Work Will Benefit You

One question drives everything I write. How do you reclaim your attention so you can find and do your Great Work?

I answer it with two tools most writers keep apart. One is the science I use every day as a physician. The other is the contemplative practice I keep every day as a person.

My writing focuses on topics like…

Most of all, I write about how to put these ideas into practice in daily life.

The Great Work Experiment

When I studied for my pain medicine boards, I used a simple method. One primary source. One secondary source. Daily review with spaced repetition. Full immersion, no shortcuts. It took me to the 99th percentile.

This site is where I test that method on bigger questions. Can the rules that worked for one exam help you find your life's work, train a calmer mind, or get a whole family moving? I run the experiment in public, one Note at a time.
